In accordance with the Law of the Republic of Belarus of July 18, 2011 “On Appeals of Citizens and Legal Entities” (hereinafter — the Law on Appeals of Citizens and Legal Entities), all the fields provided below are required and must contain the following information:

  • the “Your name” field must contain your last name, first name, middle name (if any) or the initials or full name of your legal entity (for legal entities);
  • the “E-mail” field should contain the email address to the address to which the response should be sent;
  • the “Message text” field In addition to the text of the application, it must contain: for citizens: the address of the place of residence (place of stay) and (or) place of work (study); for legal entities: the location of the legal entity, as well as the surname, own name, patronymic, if any) of the head or a person authorized to sign the application in the prescribed manner.

Additionally, we inform that if you want to receive a written copy of the response, please indicate this in the «Message text» field.

In case of non-compliance with the above requirements and in accordance with the second paragraph of paragraph 1 of Article 15 of the Law on appeals of citizens and legal entities, we have the right to leave your appeal without consideration.
